Abortion services
PPIN is dedicated to providing safe and confidential abortion services. Abortions at PPIN are performed by trained and licensed physicians. We strive to make all of PPIN's services affordable, and all of our services are confidential except as required by law. We report all cases of known or suspected child abuse to appropriate authorities.
PPIN offers two types of abortion services:
Important information for individuals 17 and under: Parental consent requirements
To reduce time in the waiting room, download your paperwork and bring it with you to your appointment. Please complete the paperwork in blue or black ink.
- Demographic Form (Hoja Demográfica). On this form, we need your name, address, phone numbers, etc., as well as your insurance or Medicaid information, if any.
- Abortion History Form (Planilla de Historia de Aborto)
Abortion services are available at the following health centers:

- Bloomington
- Merrillville
